-
Health and social care professionals (GPs, social prescribers, care staff, therapists)
-
Schools, nurseries, and youth workers
-
Voluntary sector organisations and community partners
-
Families, carers and individuals can also self-refer directly

WHO CAN REFER?

HOW DOES IT WORK?
Once a referral is received, our team will:
-
Contact you (or the individual/family) to discuss needs and preferences
-
Match you with the most suitable hub, session, or home visit option
-
Provide ongoing support to ensure participation is safe, inclusive and meaningful
Our programmes are proven to improve mental health, reduce loneliness, support learning, child development and strengthen community connections.
By referring into IMM, you’re helping people access creative experiences that transform wellbeing and bring generations together.
