Who Are We?

Our CCG programme bring together County Councils, NHS, Districts and Boroughs, Social Care and Education to build on integrated care systems and contribute to thriving community connections.

We are delivering over 100 projects across Surrey Downs area  working with schools and care homes to try and improve the lives of Surrey Downs residents, whether they be at school or in a care home.

Goals

Surrey Downs Health & Care Partnership, a collaboration of health, social care and local authority partners, has set three main goals which this project addresses: 

 

  • Help people lead healthy lives 

  • Support the mental health and wellbeing of our population

  • Support people to fulfill their potential

At the end of this 6-week project, care home staff and activities coordinators were invited to participate in a one day training course. This session ensured that the staff left with the skills they needed to use music-making tools within their own work, and to continue intergenerational sessions within the home. Project areas:
 

  • Leatherhead 

  • Dorking 

  • Banstead 

  • East Elmbridge 

  • Epsom 

  • ICP

“The is a resident living with dementia who often suffers from low mood. After the session I came to take her back to her room and her whole face had lit up. She was so happy.”

Grace and the team at IMM have been fabulous to respond to the age and ability of our children, including their songs and Makaton signing too! I would recommend a project like this and wish it had been longer than 6 weeks. We will definitely go back to visit our new friends at Garth House.’
